Required COVID-19 Guidelines and Forms
The overall safety and wellbeing of our students is of utmost importance to us. SMU requires that Programs conducted on its campus maintain certain safety and constraints associated with the pandemic and overall safety. In addition, NABA/ACAP requires certain safety measures as well. Following are some governing documents.
Listed below are several NABA/ACAP required forms. Please complete these forms (pdf fillable), save them to your drive, then email the completed forms to firstname.lastname@example.org before Check-In on Sunday, June 27, 2021. A notary will be available at Check-In.
